Corrupted LUKS/ cryptsetup on a USB

There is a bit of a back story here so please hold the snickering till the end I also preface this with Im by no means a linux expert but Im learning and well we all have to start some where right.

Installed i386 Gnome. fully encrypted on its own usb. Wouldn't boot found a previous post with the issue and used this fix.

    Code:
    cryptsetup luksOpen /dev/sda5 sda1_crypt
    (enter password)
    lvm vgchange -ay
    exit
Also edited the crypttab file and ran

    Code:
    update-initramfs -u
And here is the like for anyone who has issues booting encrypted.
    http://forums.kali.org/showthread.ph...-fails-to-boot

Now at this point every thing is working great. Till I realize I havnt restarted in like a month. No big deal shut down for the night and came back wanted to get back to playing around...

    Feel free to begin heckling.

I remembered my encrypted password no problem.
My root password for the life of my I couldn't remember. I tried no joke several hundred times.
Looking back on the situation I should of just booted the live CD and mounted the usb and fixed the password issue (not sure how but im sure a little googling could of produced an answer)

Awwwwww yet I didnt. I thought might as well re-install Kali. The idea was I installed the 32 bit version I have a 64 bit this seems like the right time to fix this.
The disk wipe failed due to I believe it was a I/O error.

I moved the USB to the desktop and tried to wipe it and write "0's" and that failed used EaseUS partition Mgr, Windows disk Manager. Moved to a live CD (bootit bare metal) partition manager to try to format it or to wipe it. Again failed. I brought it back to a Kali live CD tried to run

    Code:
    blkid
    Disk doesnt pull up.

Im sure everyone is looking at me like... this guy

Any help would be nice I would like my thumbdrive back

Thumbdrive is a CORSAIR Flash Voyager 32GB USB 3.0
    Laptop is a Toshiba Satellite
    Running Pentium T4500
    4GB RAM
It is a 64 bit system

I know its been a little while since you asked this, but no one else has answered so I thought I might - assuming you haven't figured this out already...

Have you tried deleting (and recreating) partition table using something like gparted? If you can't format it, it's likely you have some odd IO error thing happening, maybe someone else might have a clue?
