There are no usb devices attached.

recovery mode gets me a different error

This may be referring to the USB driver as well.
I realize that this macbook is not on the approved hardware list but was trying to learn what the issue is.
How to troubleshoot this?

I solved it. For some reason, the problem resolved when I eliminated refit and installed refind to the root partition of the main drive.
It turns out that by default refind installs the boot manager on its own secret partition and this is a problem for some reason. When refind is installed with './install.sh --notesp'
then the efi folder is put at the root of the main drive. And the boot loader uses the correct script and variables.
