TWRP manager can be used or you can download the latest TWRP img from the TWRP website. The latest is usually the best.

I have a current build for CM12S that can be built from our github repo that is working. Hopefully we can get a release out soon.
It would require a custom kernel build which we don't plan on doing at this time.
We have nethunter running on CM12s in testing and it should be released soon. The released version will not work with the current Kitkat (4.4.4) build.

This is a known issue with the wifi firmware (I believe) and people have had success calling the device down before disconnecting.
It's possible that your device isn't unlocked. Maybe a "fastboot oem unlock" might solve the issue. There are a lot of tools that can unlock your device if you are unfamiliar with the process...
